to Agiliti:

1. Using the Agiliti website, log in with your assigned Agiliti user name
   and password: https://vityl.agilitihealth.com/mobile
2. If you do NOT have an assigned Agiliti user name and
   password, then Call Service Number at 1-866-677-8668, and
   press 1
3. Give our name and location:
   a. Buena Vista Regional Medical Center
   b. 1525 West 5th Street, Storm Lake, IA 50588
4. Report repair concerns, additional information to have if
   possible:
   a. Name of Equipment
   b. Serial Number
   c. Model Number
5. If asked for BVRMC’s Account Number: 346800
